I downloaded it and wanted to try it but my UT2004 closes with an unspecified runtime error. I checked my UT2004.log file to see what files are needed that were not included in the download but the log file was completely empty, so I can't even help with error analysis.

A few things I can say though:
You mentioned the "thanks to" list in your readme twice and sticking to the readme: it's full of spelling errors. I'm not good at spelling myself but the readme was pretty horrible. If you have difficulties with spelling take your time if that helps you and/or get someone to help you and proof-read it when you are done. It doesn't matter much in regular internet communication, but the readme is an important part of the map even if most people simply delete it. Those who do read it (for iinstance newcomers who don't know how to install maps) might get a bad impression if it's a mess and bad spelling always conveys a certain air of unprofessionalism.
On to the map:
NEVER use spaces or other "special" characters in the mapname or a package-name!
Only use extra packages if it is really more comfortable for you. For example, if you have a certain set of trees you use in more than one map, then put them into a package. Stuff you only need in one map, or stuff that isn't important enough to have to reside in a special package should be packaged in the so-called "myLevel" package. If you need an explanation what the "myLevel" package is, ask and I'll explain it to you.
I didn't check your SM package but at least the bunker package was unnecessary (if it was my bunker) because you could have packaged it in your "myLevel" Package.
There were only two identical screenshots on the page you linked to and I can't make out much on them/it. It seems like some sort of over-head shot of the map, but I could be wrong. You should include some shots that show a closer look of your level. If you want responses from forumeers you need to post some pics anyway, so those who are too lazy to download the map, those who don't want to mess up their folders with beta material, those who don't have the game installed and those who don't even have the game at all can give you tips based on the screenshots. Those tips might not be quite as valuable as feedback from persons who have seen the map in action, but it's better than nothing and often ver helpful, especially if you want tips and comments abouts the visuals.

If you have a spare PC you can install a clean install of UT2004 on it. Patch it to the latest version and maybe even install the ECE Pack (if the ECE pack is required to run your map you have to mention that in the readme, even if most serious players have it). Just so you can try yourself if the map can be run. If you only try it on your modded UT2004 install you do of course have everything you used in the map or else you couldn't have used it.

EDIT: The game handles packagenames without their extensions so you can't have a package called generatorX.usx and one labeled generatorX.utx because the extension is ignored, which leaves the game with two packages with the same name and it will cause problems. You only have the SM package. Just saying, in case you are planning to make some textures as well. I don't know if there are also problems if the package names match the map names. No idea. I tried renaming all the files to make sure that that wasn't causing any problems (or the space in the map-name) but it still didn't work.

Is it possible for you to test the map in a clean install of UT2004 (maybe on a spare pc)?

I made a clean install of UT2004 on my old pc today so if you want me to test the map in it, post here. I don't know if you are still interested in feedback.
